Tuberculous epididymo-orchitis following intravesical BCG for superficial bladder cancer

Seventy one year old man with transitional cell carcinoma of the bladder received intravesical BCG following which he developed left testicular pain and swelling. This was treated conservatively and he received more BCG for recurrent disease 6 months later. He developed increasing left testicular swelling and induration one and a half year following his initial BCG therapy. Ultrasound confirmed a tumor-like mass, likely an inflammatory process of the left testicle. He had no systemic symptoms and his prostate felt benign. Left orhiectomy was done, it was hard with abscess formation. The discharge contained acid fast bacilli and the testicle had caseating granulomatous orchitis. M. Bulbul, A. Hijaz, M. Biaini, G. Urothelial carcinoma of the vagina six years followcystectomy for invasive cancer

Urothelial carcinoma located outside the urinary tract is rare. A variety of transitional cell neoplasia have been sporadically identified and reported in the female genital tract. These include the endometrium (1), the cervix (2), the fallopian tubes (3) and the ovaries (4) with or without concomitant bladder carcinoma. Urothelial tumors of the vagina were initially reported by Fetissof in a 76-year old woman 4 years following nephroureterectomy for transitional cell carcinoma (TCC) of the renal pelvis (%). Although Bass (6) and Singer (7) reported on vaginal TCC concomitant with bladder tumors, it was Jendresen (8) who reported vaginal TCC 9 months following cystectomy and urinary diversion in an otherwise disease free patient. The value of free and total prostate specific antigen in identifying patients for prostatic biopsy and its relationship to Gleason score and volume of prostatic adenocarcinoma

We evaluated the value of F/T PSA in 50 consecutive patients who underwent prostate needle biopsy and its relationship to the Gleason score and disease volume. Fifty patients underwent prostate needle biopsy for elevated PSA, F/T PSA <20% and abnormal digital rectal examination (DRE). 19/21 patients with positive biopsies had F/T PSA<20%. The sensitivity, specificity and positive predictive value of F/T PSA<20% was 90.5%, 31% and 48% respectively. Stratifying patients with positive biopsies to F/T PSA<10%, F/T PSA>10% and the three PSA groups, there was no relationship to either Gleason score or number of positive cores (volume of disease). Advantages of the ileal over ileo-cecal orthotopic neobladder in patients with invasive bladder cancer

Different segments of small and large bowel have been used in the construction of the orthotopic neobladder in patients undergoing cystectomy for invasive bladder cancer. We compared both ileocecal (GI) and ileal (GII) neobladders from a single institution. In 29 patients ileocecal neobladder was performed while 23 patients had ileal neobladder. In the ileocecal the cecum and ascending colon are mobilized, appendectomy is done, ileo-cecal valve is opened, ureters are implanted in tenea and a cecostomy tube is placed. In the ileal group the ureters are subserosally implanted and no suprapubic tube is exteriorized. There was no operative mortality in both groups 3 patients need intermittent catheterization. Both groups tolerate adjuvant chemotherapy and radiation therapy. 3 patients from GI developed uretero-intestinal strictures versus one from GII. Mucous production was significantly more in GI. Operating time more for GI. Both ileal and ileocecal neobladder are excellent urinary substitute and functionally comparable. The ileal neobladder offers the following advantages: 1. No mobilization of cecum and ascending colon and no appendectomy–less operating time. 2. Intact ileo-cecal valve. 3. Less uretero-intestinal strictures. 4. The value of endoscopic biopsy of the prostate or intraoperative biopsy of the urethro-protic margin in patients with invasive bladder cancer and endoscopically tumor free urethra in selecting patients for orthotopic neobladder substitution

The criteria for selecting patients with bladder cancer for orthotopic neobladder substitution includes a motivated patient, negative transurethral (TUR) prostate biopsy prior to cystectomy and/or tumor free margin biopsy at the time of cystectomy. In this report we evaluate the value of these biopsies in the endoscopically tumor free prostatic urethra. Three out of forty patient had limited-localized transitional cell carcinoma in the prostate (TCC) and one had localized adenocarcinoma. All 40 patients including the 3 with prostatic TCC had negative prostatic apical resection margin. To date, none of the patients developed urethro-neobladder recurrence. TUR biopsy of the prostatic urethra prior to cystectomy or biopsy of the prostatic apical margin at the time of cystectomy in patients with endoscopically tumor free prostatic urethra has limited value in selecting candidates for orthotopic neobladder substitution. Central line exchange policy in the ICU, 1999

Short term central venous access catheters exchange policy in the Intensive Care Unit at the American University of Beirut Medical Center consists of exchange of the catheter over a guide wire after five days (catheter sent for cultures) and removal and change of site after the second five days (Control Group). We are testing the following policy in a prospective randomized fashion: “All short term central venous access catheters will be kept until there is a change in the fever pattern to the worse where they will be exchanged over a guide wire and the tip and the transcutaneous segments are sent for quantitative cultures by the plate roll technique. If the cultures are negative, then the catheter is kept. If the cultures are positive the catheter is removed and the site is changed” (Experimental Group). We are hypothesizing, based on a review of the literature, that the experimental group will require fewer procedures, have more catheter-days, and cost less than the control group. Munchausen’s syndrome in plastic surgery—the A.U.B.M.C. experience

Munchausen’s Syndrome is a factitious illness that is characterized by one or more medical complaints dramatically presented with false and often life threatening symptoms and histories from patients who wander from doctor to doctor and hospital to hospital. Caring for these patients who frequently find their way into Plastic Surgery practices, is very time consuming because they usually fail to respond to standard modalities of treatment. The medical system is thus abused by seeking care from one or more care provider at the same time and therefore depleting expensive diagnostic procedures. This project reviews four such patients who presented to the Plastic Surgery service at the American University Medical Center of Beirut. Because of the scanty publication of such a condition in the Plastic Surgery literature this was found as a possible worthwhile contribution for study and review. I. Kaddoura.

Clinical trial: diarrhea control during enteral nutrition

A quality care clinical study where Guam fiber enriched enteral feeding is compared to standard feeding formula. Patients are entered into the study if they have been on enteral nutrition with three or more liquid stools per day grossly in excess of 300cc volume. Patients with short bowel syndrome, acute bacterial infection or hyperthyroidism are excluded. Patients are given 25-35 K Cal/kg/day enteral feeding through pump assisted system. Hemorrhagic shock decreases alanine absorption in the rat jejunum

Little is known about the absorption of nutrients by the gut following severe hemorrhage. Single pass perfusion technique is used to measure net alanine absorption and water transport in an isolated jejunal segment of adult rats. Shock is induced by bleeding the rat down to 60% of its blood volume for one hour. Severe hemorrhage causes a marked decrease in alanine absorption that persists during the shock state. Resuscitation with blood and saline restores only a part of this absorptive defect. Addition of calcium channel blockers to the resuscitation regimen returns the absorption of alanine to control values. Resuscitation from hemorrhagic shock is associated with a persistent defect in absorption of amino-acids in the rat jejunum. Calcium channel blockers correct this defect by either improving gut blood flow or preventing reperfusion injury and therefore it may have a major therapeutic role in the treatment of hemorrhagic shock. The effect of septic shock on the absorption of aminoacids in the rat jejunum

Experimental model for septic shock has been developed in the adult rat. Single pass perfusion technique is used to measure net alanine absorption and water transport in an isolated jejunal segment. Mesenteric blood flow is measured by a transducer placed on the portal vein artery. Septic shock is induced by injecting the rat with 20mg/kg lipopolysaccharide. Severe sepsis decreases alanine absorption that persists and worsens despite of resuscitation with fluids and calcium channel blockers. Addition of norepinephrine to the regimen restores the absorptive capacity of the gut to the control value. These findings may have a major therapeutic implications in the resuscitation of shock and the pharmacologic manipulation of the gut to restore it absorptive capacity for nutrients. A.M. Taha, C.F.
