The mechanism of action of zidovudine and alpha interferon in treating HTLV-I associated adult T-cell leukemia/lymphoma (ATL) We showed that the combination of zidovudine (AZT) and alpha
interferon (IFN)
is a potent treatment of HTLV-I associated ATL, an aggressive T cell malignancy.
We investigated the mechanism of action of AZT and IFN using HTLV-I positive
and negative T cell lines. No major modification was observed except at
supra pharmacological doses. Moreover, these little in vitro effects
were less pronounced in ATL derived cell lines compared to control. This
treatment did not induce any significant modification of the expression
of the apoptosis regulatory proteins bcl-2 and p53. Interestingly no in
vitro cytotoxic effect was observed on fresh leukemic cells derived
from one acute ATL patient at diagnosis despite achievement of in vivo
complete remission using the same therapy. These results suggest that the
therapeutic effect of this combination is not through a direct cytotoxic
effect but suggest in vivo modulation of the viral expression and

The mechanism of action of Arsenic trioxide in combination with alpha interferon in HTLV-I transformed cells We
have shown that the combination of arsenic trioxide (As) and interferon-a
(IFN)
synergize to induce cell cycle arrest and extensive apoptosis in HTLV-I
transformed cells. We investigated the mechanism of action of this combination
on pathways central to the control of cell proliferation and programmed
cell death (apoptosis). We demonstrate here that the combination of As
and IFN specifically targets the transcription factor NF-kB
pathway an essential pathway of cellular transformation by HTLV-I. This
treatment specifically inhibits the transfer from the cytoplasm to the
nucleus of the NF-kB
subunit p65 concomitant with an increase in the level of IkB-a
protein.
IFN and As treatment did not induce any significant modification of the
expression of the Fas antigen (CD95), bcl-2, p53, IkB-band
NF-kB
subunit p50 proteins. Moreover, this treatment induced apoptosis through
a caspase-dependent as well as a caspase-independent pathways. A.A.

Effect of age and gender on PTH dynamics We
systematically evaluated the impact of age and gender on the Ca-PTH axis
using dynamic stimulation with calcium-citrate infusions in 48 normal healthy
individuals. There was no effect of aging and gender on any of the parameters
characterizing the Ca/PTH axis. Healthy aging does not appear to be accompanied

Bone turnover is truly circadian While
an increasing body of evidence demonstrates that bone resorption markers
follow a diurnal rhythm, it is still unclear whether these reflect endogenous
changes or are dictated by external factors. We evaluated the diurnal rhythm
of N-telopeptides cross-links (N-Tx) in 8 healthy white male volunteers.
The first 24 hours spent under baseline conditions were followed by 24-48
hours of constant routine conditions (CR, enforced wakefulness in the strict
semirecumbent position, with the consumption of hourly snacks). For both
the baseline day and the CR, circadian rhythm parameters were estimated
from a one harmonic regression model that was applied to the N-Tx data.
During baseline conditions, N-Tx followed a diurnal rhythm with an average
amplitude of 17.9 nM BCE/mM Cr and a Tmax at 5:45 a.m. This
rhythm was preserved under CR with a slight shift in its average upwards
and an increase in its amplitude to 23.8 nM BCE/mM Cr and a Tmax
at 5:15 a.m. In conclusion, N-Tx levels exhibit a diurnal rhythm that persists
during a CR, thereby confirming that a large component of this rhythm is

Effect of posture on PTH diurnal rhythm The most significant hazards facing astronauts
who embark on space flight missions is the significant loss of skeletal
mass. The mechanism (s) for such losses are unclear. In this project, we
used the -5 head down tilt (constant posture CP, a simulation of microgravity)
to evaluate the impact of weightlessness on mineral and skeletal homeostasis.
During baseline conditions, PTH levels followed a bimodal diurnal rhythm
with an average amplitude of 6 pg/mL. This rhythm was preserved under CP
conditions, albeit with different characteristics, thus confirming its
endogenousnature. During the CP, PTH rhythm was unimodal and had a blunted
amplitude measured at 5 pg/mL with a broad plateau from 6 p.m. to 6 a.m.

Bone mineral density in the US: a national random survey At the present there are no guidelines for BMD reporting.
In this national random survey we evaluated the practice of BMD reporting
in the US, using the listing of densitometry centers registered in the
National Osteoporosis Foundation database. Of the 1200 questionnaires mailed,
22.5% were completed and returned. The returned questionnaires reflected
the geographical distribution of the densitometers in the US. Both spine
and hip BMD were routinely measured in 71% of the centers and were expressed
as T scores by 90% of centers. The WHO working group definition of osteoporosis
was used by 65% of the survey respondents and was used as the sole criterion
to make treatment recommendations by 33% of the respondents. Fracture risk
was not mentioned in 31% of reports. There was geographic variation in
reporting fracture risk and variations among specialties in BMD reporting
and clinical recommendations. National guidelines for the inclusion of
validated and clinically relevant information and recommendations in bone

Calcium content of food in Lebanon: accuracy and reproducibility Osteoporosis is on of the commonest public health problems
of modern medicine. Adequate calcium intake is essential to optimize skeletal
health in childhood, adolescence and maintain it during adulthood. The
National Institute of Health Consensus Panel put forward recommendations
for calcium intake at various age groups. Calcium can be provided through
supplements or through natural food sources, the latter a cheaper and in
some ways more optimal source of calcium as it also provides other essential
nutrients and vitamins. Unfortunately, the calcium content of many food
products in Lebanon is unavailable. Furthermore, it is unclear how accurate
the information provided on the few food items that do carry a label is.
It is therefore difficult at this point to make specific recommendations
on how to optimize an individual’s calcium intake using food products in
Lebanon. The aim of this investigation is to evaluate the accuracy and
reliability of calcium content of commonly used food products in Lebanon

Prevalence of vitamin D insufficiency in school children in Beirut: safety and efficacy. Vitamin D is essential for growth and development in children.
Vitamin D deficiency results in rickets and osteomalacia. Evidence supports
that the deleterious effect of subclinical vitamin D deficiency results
in poor bone formation, mostly in adults. There is a positive relationship
between vitamin D intake and peak bone mass with a negative effect of subclinical
vitamin D insufficiency on skeletal development in adolescents. The attainment
of suboptimal bone mass during adolescence may be a major reversible risk
factor for osteoporosis in adulthood. The study objectives are to determine
the prevalence of vitamin D insufficiency in school children in Beirut,
characterize vitamin D insufficiency into deficiency or insufficiency,
develop a predictive model for vitamin D levels in children, and to validate
the biological relevance of vitamin D insufficiency in these children.
