Dermoid cyst of the external auditory canal: a case report and review of the literature Dermoid
cysts are congenital lesions of the head and neck area. This paper discusses
the unusual presentation of such a cyst in the ear canal. Diagnostic work-up

Enophthalmos in asymptomatic sinusitis: a variant of silent sinus syndrome Enophthalmos
has been described in conjunction with several intraorbital and periorbital
pathologies, with trauma being the commonest causative factor. This entity
is often associated with symptoms alluding to its etiology; however, the
condition becomes more challenging when the patient with enopthalmos presents
with no other symptoms. This has been known as the Silent Sinus Syndrome,
which is characterised by spontaneous enophthalmos, absence of orbital
floor, hypoplastic maxillary sinus, minimal evidence of infection and absence
of prior trauma or surgery. We are drawing attention to a variant of this
syndrome having peculiar radiologic and pathologic parameters, namely intact
orbital floor and normal sized infected maxillary sinus. The report also
includes a review of the literature, differential diagnosis and surgical

Neurogenic tumors of the neck The
neck is an important area containing a wide variety of structures that
can be at the basis of congenital, inflammatory and neoplastic lesions.
The differential diagnosis of a neck mass is quite wide, presenting a challenge
to the physician. Neurogenic tumors of the neck form 20-36% of parapharyngeal
space tumors which include less than 1% of all head and neck tumors. Apart
from paragangliomas, neurilemomas are the most commonly encountered neurogenic
tumors in the neck. These are benign, usually aymptomatic until they attain
a large size. The vagus nerve is mostly the origin but theoretically, the
tumor can arise from any nerve sheath. Two peculiar variants are the facial
nerve and jugular foramen neurilemomas. On the other hand, the sympathetic
chain can be the origin of another interesting rare tumor, namely ganglioneuroma.
This comes third on a line of cytomaturation of malignant precursors: neuroblastoma
and ganglioneuroblastoma. Ganglioneuroma is benign and solitary, yet multiplicity
should be kept in mind, necessitating screening of the whole sympathetic
chain and even the adrenals. In this presentation, we will go over an example
of each of these two entities: i.e. neurilemoma and ganglioneuroma elaborating
on our experience at AUB Medical Center in diagnosis and managing these

Fungal sinusitis in immunocompetent patients endoscopic management Fungal
sinusitis in immunocompetent patients is a very rare challenging entity
to the otolaryngologist which can be easily missed in the absence of a
high index of suspicion. We conducted this retrospective study to highlight
the risk factors predisposing to this disease and the best method to diagnose
it. We reviewed 110 cases operated on at AUB Medical Center between 1960
and 1994 for a presumed diagnosis of chronic sinusitis. We found 5 cases
of fungal sinusitis among the reviewed patients, with an incidence of 4.5%
whose ages ranged between 30 and 47 years with a mean of 37.5 years. All
patients were immunocompetent. The major contributing risk factors were
unilaterality of the disease and prolonged use of topical steroids. Four
of these cases were managed by endoscopic sinus technique with excellent
results at up to 2 years follow up. The authors stress the importance of
maintaining a high index of suspicion in patients with predisposing factors
and the necessity of alerting the pathologist for the possibility of fungal

Post adenoidectomy inflammatory pseudo tumor A
rare complication following adenoidectomy. Clinical features and physical
findings are stressed, and management discussed. U. Hadi, M.

Sarcoidosis of the sinonasal tract Granulomatous
diseases of the sinuses are unusual. Sarcoidosis rarely involve the nose.
Sarcoidosis involving the sinonasal tract can resemble any patient presenting
with chronic recurrent sinusitis. Clinical presentation and subtle physical
findings are discussed that can alert the clinician to this rare entity.

Prospective analysis of microorganisms in CSOM by culture and PCR analysis A
prospective analysis of 50 ear samples with CSOM for studying and analysing
the pattern of microorganisms involved in CSOM using routine culture techniques
and compared against PCR. PCR has proven to be a highly sensitive technique
in detecting pathologic microorganisms where routine cultures have failed.

Valsalva-induced thymic cyst This
is a case report of a child with a suprasternal neck mass that would reveal
itself from the upper mediastinum upon performing the valsalva maneuver.
Diagnostic work-uas well as pathologic examination of the removed tissue
proved it to be a thymic cyst. The interest of the case is in its unusual
presentation as well as in the surgical removal. This paper has been accepted
for publication in Otolaryngology-Head and Neck Surgery Journal.

Endoscopic repair of choanal atresia Choanal
atresia is a fairly common congenital anomaly of the nose. Bilateral cases
present early in life, and are difficult to diagnose and manage compared
to unilateral cases. Different surgical approaches have been described.
We are presenting three cases (two cases with bilateral, one case with
unilateral atresia), that were managed endoscopically. The surgical technique
will be illustrated. Adjuvant surgical procedures will be described. U.

Extensive laryngotracheal necrosis following open heart surgery A
case report of unusual and fatal complications following open heart surgery.
The rarity of the case is discussed and the predisposing factors highlighted.
