RESEARCH PROJECTS

Sinplified approach to egg donation using synthetic estrogen and progestogen

Our preliminary experience with oocyte donating using a simplified approach is reported. Endometrial preparation using synthetic estrogen and progestogen instead of the natural preparations was done. The pregnancy rate was similar to that reported in the literature. A. Abu-Musa.

Effect of follicular on sperm survival test

Sperm survival test is used to evaluate the fertilizing capacity of sperm. Washed spermatozoa were incubated for 72 hours in Ham’s F-10 medium alone or in Ham’s F-10 medium lus follicular fluid. Sperm moyility was assessed every 24 hours. There was no difference in sperm survival between the two culture media, suggesting no beneficial effect of follicular fluid on sperm survival. A. Abu-Musa.

Assessment of ovarian reserve in patients with autoimmune disease

Ovarian reserve using clomiphene challange test is being evaluated in patients with autoimmune disease (e.g. Hashimoto’s disease) and compared to normal subjects. A. Abu-Musa.

Placental pathology in patients with meconium stained amniotic fluid: Correlation with cord pH apgar score and fetal heart activity

Fetal outcome assessed by apgar score and cord pH were correlated with the pathologic findings of placentas having meconium-stained amniotic fluid. A significant vascular abnormality was seen in placenta of pregnancies with an adverse outcome. It is concluded that subacute and chronic meconium discharge is associated with vascular lesions of the placenta and may predict and adverse pregnancy outcome. A. Abu-Musa and H. Kaspar.

Internal podalic version of the malpositioned second twin

The mode of delivery of the non vertex second twin remains very controversial, though the current trend of practice is in favor of the use of cesarean delivery. We planned to review our ten year experience at the AUB Medical Center retrospectively in order to evaluate the usefulness and safety of internal podalic version followed by vaginal breech extraction in the management of twin gestations with a malpositioned second member. J. Awwad, G. Azar, A. Khalil, M. Seoud and J Sawaya*.

Photodynamic endometrial ablation in women with dysfunctional uterine bleeding

Dysfunctional uterine bleeding is a common gynecological complaint of perimenopausal women and is a major cause of hysterectomy nowadays. Because a hysterectomy is a major surgical procedure that involves high morbidity and expense, we are investigating the use of a minimally invasive approach that can achieve the same final clinical outcome. The technique consists of destroying the endometrium by using photodynamic therapy in an office setting, precluding the need for anesthesia. The photosensitizers used in this clinical trial are Photofrin and Benzoporphyrin derivatives. The light source is an argon pumped dye laser. J. Awwa, L. Bhatta*, P. Dwyer*, T. Hasan*, R. Anderson* and I. Schiff*. (Supported by the Harvard Center for Minimally Invasive Therapy at Harvard University.)

A comparative cross-over study of the use of FSH vs. HMG for ovarian stimulation in an IVF program

In this prospective crossover study, the use of pure follicle stimulating hormone (FSH) was compared with human menopausal gonadotropins (HMG) for controlled ovarian hyperstimulation following long gonadotropin-releasing hormone suppression in an IVF-ET program. The effects of both stimulation protocols on folliculogenesis, oogenesis, and steroidogenesis during ovarian stimulation are evaluated. J. Awwad, J. Sawaya* and T. Toth*.

The efficacy of a 12-month course of Synarel on pain relief in patients with endometriosis

Endometriosis is a gynecological disease that affects approximately 10% of women. It is a significant cause of infertility and pelvic pain. Relief of endometriosis-associated pelvic pain can be attained by medical treatment. However, this approach is often associated with recurrence of symptoms. We planned to evaluate the long-term clinical results in terms of symptom relief and recurrences in women who received the gonadotropin-releasing hormone agonist Synarel for as long as 12 months. J. Awwad, T. Toth*, R. Sayegh*, I. Schiff* and J. Finkelstein*.

Description and characterization of an animal model for endometriosis: the SCID mouse

Endometriosis is a significant cause of infertility and pelvic pain in women. Understanding of this condition has been limited by the absence of an ideal animal model for the study of the disease. Human endometrial tissues were implanted successfully unto the peritoneum of SCID mice. This basic molecular research aims at characterizing the morphological, histological, biochemical, and immunological properties of human endometrial implants in SCID mice. J. Awwad, R. Sayegh*, X. Tao*, T. Hassan* and K. Isaacson*. (Supported by the Center for Innovative Minimally Invasive Therapy at The Massachusetts General Hospital.)

An open multicenter study of chronic low dose administration of recombinant follicle stimulating hormone (r-FSH) for anovulation associated with polycystic ovary syndrome

One conventional treatment modality of Polycystic Ovary Syndrome associated unovulation is by ovarian stimulation using follicle stimulating hormone (FSH). FSH has been obtained thus far through a complex purification process from the urine of menopausal women. As a result of a major breakthrough in biotechnology, recombinant FSH can now be prepared in the laboratory. This multicenter randomized controlled prospective study was designed to evaluate the clinical efficacy of recombinant FSH compared with urinary FSH for ovarian stimulation in women with polycystic ovary syndrome. J. Awwad, et al. (Supported by Serono.)
