A simple technique for contralateral left bronchial intubation in children undergoing right thoracotomy or thoracoscopy

The report describes a simple technique for blind contralateral left bronchial intubation in young children undergoing right thoracotomy or right video-assisted thoracoscopy. Following tracheal intubation, the tube was rotated 90 degrees counter-clockwise, and the neck of the child was tilted to the right. Thus, the curve of the tube was concave to the left, causing the tube to slide along the left wall of the trachea towards the left main stem bronchus. A. Baraka. Succinylcholine resistance in a patient with juvenile hyaline fibromatosis

Succinylcholine resistance is a rare phenomenon which may occur in patients with elevated plasma cholinesterse, or in myasthenic patients. However, the present report describes unusual resistance to succinylcholine in a patient with juvenile hyaline fibromatosis (JHF), who had normal cholinesterase values, as well as normal neurologic and neurommuscular functions. Correlation of the end-tidal PCO2 (ETCO2) during laparoscopic surgery with the pH of the gastric juice—Influence of the H2-blocker nizatidine

During laparoscopy, the pH of the gastric juice is significantly decreased. The decrease in gastric pH is inversely related to the increase of ETCO2. The pre-operative administration of the selective H2-blocker nizatidine can prevent the increase of gastric acidity, and can even result in a paradoxical increase of gastric pH. A. Baraka, S. Intravenous lidocaine for prevention of pain on injection of rocuronium

Intravenous administration of lidocaine 1.5 mg/kg prior to rocuronium-propofol sequence minimizes the burning pain on injection, probably by a local anesthetic action on the sensitive vein wall. Lidocaine may also act centrally to deepen the level of anesthesia, and improve the conditions of tracheal intubation. A. Baraka, S. Taha and S. Haroun-Bizri. The self-inflating bulb (SIB) versus end-tidal capnography for confirming tracheal intubation in the parturient undergoing cesarean section

The self-inflating bulb (SIB) was associated with a high incidence of false negatives in confirming tracheal intubation in the parturient undergoing cesarean section. The false negatives can be attributed to decreased functional residual capacity by the gravid uterus, which is further decreased by the SIB-induced subatmospheric pressure. This can result in airway closure, with a consequent delayed reinflation of the SIB. In contrast, capnography was positive in all parturients, whether the SIB reinflated rapidly, slowly, or even did not reinflate at all.
