Dear AUB Faculty/Staff,
We would like to inform you that
you and your family will be covered by SNA–MONDIAL ASSISTANCE’s
“TRAVEL-EXTRA” travel insurance and assistance plan during
your stay outside Lebanon.
TRAVEL-EXTRA plan covers
emergency medical expenses abroad up to
$100,000 per person per period of coverage.
Emergency medical expenses include
sickness or accidents i.e. sudden, serious,
unpredictable/unexpected illnesses or accidents for which
treatment cannot be delayed such as a child has very high fever or
serious accident.
However, travel insurance does not
cover regular check ups or pre-existing conditions such as
hypertension, diabetes, heart disease, etc.
For minor emergency expenses, the
insured is not necessarily compelled to contact the call center.
S/he can make the necessary payment and will be reimbursed in
Beirut as soon as s/he sends all the necessary documents (Report
from the attending physician, bills, declaration, etc.) to SNA.
Kindly note that minor emergency
expenses are those resulting from emergency sickness or accidents
which require immediate medical attention and not exclusively
hospitalization.
In case of a medical claim
requiring admission to the Emergency Unit or to the hospital, the
insured should contact the call center of MONDIAL ASSISTANCE
which is available 24/7 and is located in Paris immediately or
within three days at the latest following the occurrence of the
event:
Tel:-+33-1-42-99-64-90
Fax:-+33-1-42-99-03-00
Email:
medical.assistance.fr@elviatravel.com
Please provide the call center the
necessary information:
- Full name
- Policy
number
- Location
- Telephone
number where the Insured can be reached
- The problem
faced
The call
center will provide the insured a claim number and will do the
necessary arrangements and follow up.
The $60 deductible paid by the
insured when discharged is reimbursed by the University upon
return.
Moreover, the cost of the
international telephone calls made to MONDIAL ASSISTANCE is
reimbursed by the insurer in Lebanon provided that original bills
are submitted.
In case of loss, theft, damage or
delay of luggage, the insured should make all the necessary
arrangements with the airline company in order to have all
original documents recognizing the responsibility to be submitted
to EHSRM in Lebanon. For these claims the insured should not
contact the call center of MONDIAL ASSISTANCE but should
present the required documents in
Lebanon in order to be reimbursed.
If it is a yearly
cover, the policy is valid for multiple trips as long as
each stay outside Lebanon does not exceed 90 consecutive days.
If your stay will exceed this period, you are kindly
requested to indicate this on the
application form. If more than a 90 consecutive-day-stay was not
planned, kindly notify EHSRM a week prior to the expiration of the
90 consecutive days in order to issue the corresponding extension.
Kindly refer to:
-
The
Application Form (this form can be filled and sent as an
email attachment).
-
The
General Conditions
including a summary of the guarantees on pages 7 and 8 and
exclusions on pages 5, 14, 18, 20, and 22.
Kindly let us know if we can be of
more assistance in this regard.
Sincerely,
Aline Assaf Bassil
